Funding Guidelines and Application
ORDC requires a Rail Funding Application to be completed prior to the provision of any grant or loan assistance. Please contact ORDC staff with any questions about the Funding Guidelines and Application.
Use the following links to reference specific forms that may be required to complete the application:
ORDC Disclosure Release March 2011.pdf: Required for all projects.
Declaration of Material Assistance: Required for projects requested by non-public entities who have or will receive over $100,000 in state funds in any year. Use the "DMA for funding and business contracts" form.
Internal Revenue Service W9 Form: Required for recipients who are not already in the state's vendor database.
Vendor Information Form: Required for recipients who are not already in the state's vendor database. Scroll to the bottom of the linked site and select the Vendor Information Form (OBM-5657).
