PLEASE READ BEFORE SUBMITTING:
By submitting the below participation form and a resolution/ordinance for the Winter-Use Salt Contract (018-16) you will be bound to purchase at least 90% of the salt quantities requested. Conversely, you will be able to purchase up to 110% of the salt quantities requested. After the deadline noted below, you cannot change your position or back out of this contract at any time. NO EXCEPTIONS. The Winter salt contract will be valid from November 1, 2015 through May 31, 2016. Orders may be placed beginning November 1st. Once an order has been placed, your salt supplier will have seven (7) calendar days to deliver the balance of your order. You will be responsible for placing all orders and making payments directly to your salt supplier. You will also be responsible for settling all contract disputes between your Political Subdivision and your awarded salt supplier.
 
The Winter-Use Contract will be put out to bid approx. May 19th and bids will open around June 16th. We will share the bid results by email shortly thereafter.
 
This participation form AND your Resolution/Ordinance must be submitted by no later than 5:00PM on MONDAY, MAY 18th. The Department will NOT accept your salt request if you fail to attach a completed Resolution or Ordinance to the form (first field on the form). NO EXCEPTIONS. After May 18th, you will not be given an opportunity to withdraw or reduce your requested tonnage from the contract.
 
PLEASE NOTE: YOU MUST ATTACH AN APPROVED RESOLUTION/ORDINANCE TO YOUR FORM. EVEN THOSE WHO HAVE ALREADY SUBMITTED ONE FOR THE SUMMER CONTRACT FORM MUST RE-ATTACH IT TO THEIR WINTER FORM. YOU MAY USE THE SAME RESOLUTION/ORDINANCE FOR EITHER CONTRACT.
 
HOW TO VERIFY THAT YOUR FORM WAS SUCCESSFULLY SUBMITTED
 
It is your responsibility to verify that your submitted form has been received and that the tonnages you have requested are correct. We have provided a link below that you can use to check to make sure your form was submitted properly and that all information is correct.

Please note: This link is set to update every night to include all of the data submitted that day, so your submission will NOT immediately appear on this list when you click submit. Please check back after 11:00am the following day to make sure everything submitted is correct.
 
MAKING CHANGES TO ALREADY SUBMITTED FORMS
 
If you need to make any changes to your information once you have already submitted the form you must do so by no later than 5:00 PM on Monday, May 18th.
 
To make changes you can simply re-submit the form and it will automatically overwrite all of the information on the form you had previously submitted. Therefore, you must fill out the form in its entirety in order for your changes to be made. Please check back the next day after 11:00am to ensure that your changes were made.
