The Ohio Unified Certification Program provides "one-stop shopping" for firms seeking Disadvantaged Business Enterprise certification. The program's DBE certifications will be honored by all U.S. DOT recipients (i.e., ODOT, transit systems, airports) in Ohio.

 Applying for DBE Certification - Ohio Firms

 

If your firm is based in Ohio, please complete the steps below to apply for DBE certification.
  1. Download the Uniform Certification Application and the Personal Net Worth Statement.
  2. Open the downloaded application, fill it out (and save it), and gather your supporting documents, including the Personal Net Worth Statement.
  3. Download the NAICS Code/Descriptor Request Form.
  4. Open the downloaded form, fill it out (and save it), and gather your supporting documents.
  5. Combine, into one file, the supporting documents for your Uniform Certification Application and your completed NAICS Code/Descriptor Request Form along with its supporting documents.
  6. Click the Submit Online button below to open the Certification Application Attachment form.
  7. Complete the Certification Application Attachment form, uploading your completed Uniform Certification Application and your supporting documents file, and following the steps shown to electronically sign and submit the form.
  8. Be sure to save a copy of everything you submitted for your files.

 Applying for DBE Certification - Out-of-state Firms

 

​If you firm is not based in Ohio, please complete the steps below to apply for DBE certification in Ohio.

  1. Download the Interstate Certification Request form.
  2. Open the downloaded form, fill it out (and save it), and gather the requested documents.
  3. Print the completed form, sign it, scan it, and email the signed form along with its accompanying documents to DOT.SDBE@dot.ohio.gov.
  4. Be sure to save a copy of everything you submitted for your files.
 

 Forms for Certified DBE Firms

 
Annual Declaration: Please complete the steps below to submit your Annual Declaration.
  1. Obtain a signed copy of your complete Federal business tax return for the past year (including all schedules and forms). If your firm is a sole proprietorship or an S corporation, you will also need a signed copy of your complete Federal individual tax return for the past year (including all schedules and forms).
  2. Click the Submit Annual Declaration button below to open the Annual Declaration form.
  3. Complete the Annual Declaration form, uploading your tax returns and following the steps shown to electronically sign and submit the form.
  4. Be sure to save a copy of everything you submitted for your files.

NAICS Code/Descriptor Request Form: To request additional NAICS codes and/or descriptors, please complete the steps below.

  1. Download the NAICS Code/Descriptor Request Form.
  2. Open the downloaded form, fill it out (and save it), and gather your supporting documents.
  3. Print the completed form, sign it, scan it, and email the signed form along with its supporting documents to DOT.SDBE@dot.ohio.gov.
  4. Be sure to save a copy of everything you submitted for your files.

Once ODOT receives your request, we try to let you know within 30 days if the request is complete. If it is, we try to make a decision on your request within 90 days from the date of the original submission. If the request is not complete, we will let you know what is missing. The review process restarts once you submit the additional information.
