Obtain a signed copy of your complete Federal business tax return for the past year (including all schedules and forms). If your firm is a sole proprietorship or an S corporation, you will also need a signed copy of your complete Federal individual tax return for the past year (including all schedules and forms).

Complete the Annual Declaration form, uploading your tax returns and following the steps shown to electronically sign and submit the form.
Be sure to save a copy of everything you submitted for your files.
NAICS Code/Descriptor Request Form: To request additional NAICS codes and/or descriptors, please complete the steps below.
- Download the NAICS Code/Descriptor Request Form.
- Open the downloaded form, fill it out (and save it), and gather your supporting documents.
- Print the completed form, sign it, scan it, and email the signed form along with its supporting documents to DOT.SDBE@dot.ohio.gov.
- Be sure to save a copy of everything you submitted for your files.
Once ODOT receives your request, we try to let you know within 30 days if the request is complete. If it is, we try to make a decision on your request within 90 days from the date of the original submission. If the request is not complete, we will let you know what is missing. The review process restarts once you submit the additional information.
