For a variety of processes, standard forms have been developed to assist Researchers and Subject Matter Experts in preparing items to submit to IRIS. These forms are designed to help document necessary information to allow for more efficient processing.

The forms have been divided into categories based on the individual completing the document. If you are conducting a study for ODOT, interested in submitting a proposal, or responding to a call for student study problem statements, copies of standard forms for you are under the Researchers section. If you providing technical oversight to a research study, copies of standard forms for you are under the Subject Matter Experts (SME) section. ODOT personnel who want to submit an idea for a new project can find those forms in the section for SMEs.
