Important Information for Option A (“Hold Course at ODOT Regional Site”)

Prior to the course, each registrant/participant from your agency will be required to sign and submit a copy of this form. The original signed/dated forms for all registrants should be mailed in one envelope to Ohio LTAP (attention: Sarah Welsh) at least two weeks before the course date.
Important Information for Option B (“Hold Course On-Site at Local Agency”)

These diagrams illustrate the setups for skills exercises that are an integral part of the course. In order to host the course on-site, your agency must be able to provide a safe site area of sufficient size (approximately 4 acres) for the driving skills portion of the training. In addition, your agency must be able to provide and set up traffic cones/devices as needed for the various driving skills exercises, per the Layout Diagrams.
An insufficient site area could affect the quality of the training, or result in a need to cancel the course.
