If your address has changed, please let us know.

You can contact us in the following ways:

1. Fill out the Update Your Mailing Preference form on this screen: (or)

  • Provide your OT2C number (located on the LTAP newsletter label)
  • Enter the new information for Street Address, City, State, Zip, County, Phone or Fax Number 
    •

    2. Call LTAP to let us know by phone: 877-800-0031 (toll free) or 614-387-7359 (or)

    3. Fill out the LTAP Address Change form (below) and send to Ohio LTAP by fax (614/466-2120) or mail (LTAP 1980 W.  Broad St., Columbus, OH  43223).
