Please use the form below to select options and provide information to the Ohio Department of Transportation about the location and incident. 
 
 Providing us with detailed and precise information regarding the reported location – especially the county, route and time – is important and helpful to the process.
  
Consult ODOT's Official Transportation Map Online or other online mapping resources such as Bing Maps, Google Maps or Mapquest to help identify location details.

All ODOT maintained roadways (Interstate, US, or State Routes - use the selector above) have a route number. Any local street, avenue, boulevard, etc. with a name (and no other route number designations) is NOT under ODOT’s maintenance jurisdiction and notices/claims should be made with the municipality (city, village, or county) where the incident occurred.
Once your report is received by the ODOT District, you will receive an email or phone call letting you know who has maintenance jurisdiction:
1. ODOT
2. ODOT Contractor
3. Municipality (county, city or village)
With information to file a claim with the appropriate party. Please allow one (1) to two (2) business days for ODOT District to reply to your report. Thank you.
