Certification – Supplement 1092 and/or Supplement 1093

Material shall be furnished by a certified plant as per Supplement 1092 and/or 1093. A list of certified suppliers is available at the OMM website http://www.dot.state.oh.us/Divisions/ConstructionMgt/Materials/Pages/default.aspx

2. Sampling and/or Inspection Requirements:

Certified materials should arrive on a TE-24 certified materials shipping document. The certified supplier's bill of lading detailing the materials delivered in that shipment should be attached to the TE-24 document. If the material fulfills the requirements it may be used immediately. If the material does not fulfill the requirements, reject the material and notify the OMM.

Quantity and dimensions shall be verified by project personnel.

Comparison of bill of material with the materials shipped should be completed.
