Mailing the documents is another option but please note that hand-delivered documents will not be accepted.

The Ohio Unified Certification Program provides "one-stop shopping" for firms seeking Disadvantaged Business Enterprise and Airport Concession Disadvantaged Business Enterprise certification. The program's DBE and ACDBE certifications will be honored by all USDOT recipients and subrecipients (i.e., ODOT, transit systems, airports) in Ohio.

For certification as a Small Business Enterprise with ODOT, see ODOT's Small Business Enterprise page. Other USDOT recipients may have their own Small Business Enterprise certifications.
Online Form Troubleshooting
If you scanned your document(s) and the file is larger than 16 mb, try rescanning in black and white and not more than 300 dpi resolution. Many scanners are set by default to scan in full color or grayscale, which look better than black and white but make for large files. Refer to your scanner's manual for instructions on how to change the scan settings.

Applying for Certification - Ohio Firms
- Download the Uniform Certification Application / Solicitud para certificación unificada and the Personal Net Worth Statement.
- Open the downloaded application, fill it out (and save it), and gather your supporting documents, including the Personal Net Worth Statement.
- Download the NAICS Code/Descriptor Request Form.
- Open the downloaded form, fill it out (and save it), and gather your supporting documents.
- Click the Submit Certification Application button (below). On the page that appears, enter your first name, last name, and email address. Double-check that you typed your email address correctly, then click Submit. You will see “Package created successfully!”
- Check your email for an email from eSignLive (signers@signer-gov.esignlive.com). In that email, click "Go to Documents".
- Read the disclosures and click the Accept button at the bottom.
- Complete the Uniform Certification Application Submission Form and upload your signed and notarized application, NAICS Code/Descriptor Request Form, personal net worth statement(s), and other required and optional supporting documents. The first upload slot is for the Uniform Certification Application. The second upload slot is for the NAICS Code/Descriptor Request Form. You can attach all of your required and optional supporting documents in one PDF or as separate PDFs (in whatever way is easiest for you). If there isn’t enough space to attach all of your supporting documents, please submit the remaining documents using the Uniform Certification Application Additional Supporting Documents form.
- Finished uploading documents? Click Confirm. Your form is now complete. (If you get a message about missing attachments, it just means that you didn't use all the available upload slots--this message can be dismissed by clicking the Yes button.)
- Please download a copy of the completed Uniform Certification Application Submission form and save it in your files, along with a copy of everything else you submitted with your application.
The UCP's goal is to acknowledge certification applications within seven days of receipt. Please allow 120 days for a final certification decision.

Applying for Certification - Out-of-state Firms
Forms for Certified DBE/ACDBE Firms
Obtain a complete, signed copy of the following:
- the most recently filed Federal tax return for the certified firm, including all schedules
- the most recently filed Federal tax return for each affiliate (if any), including all schedules
- Extension(s) of time to file (if the certified firm or any of its affiliates are under filing extensions for the current year)
- The most recently filed personal Federal tax return(s) if, for the firm or any of its affiliates, profit or loss is reported to the IRS on Schedule C of Form 1040
Click the Submit Annual No Changes Declaration button below. On the page that appears, enter your first name, last name, and email address. Double-check that you typed your email address correctly, then click Submit. You will see “Package created successfully!” The Annual No Changes Declaration form will be emailed to you.
Check your email for an email from eSignLive (signers@signer-gov.esignlive.com). In that email, click "Go to Documents".
Read the disclosures and click the Accept button at the bottom.
Complete the Annual No Changes Declaration form and upload the required documents.

Submit Annual No Changes Declaration
Declaration of Changes in Circumstances or Firm Information: Please complete the steps below to submit your Declaration of Change.
Preferably on letterhead, create one or more documents to explain what changed and the effective date of each change. You will need to attach these to the form along with any other supporting documents.
Click the Submit No Changes Declaration button below. On the page that appears, enter your first name, last name, and email address. Double-check that you typed your email address correctly, then click Submit. You will see “Package created successfully!” The Declaration of Change form will be emailed to you.
Check your email for an email from eSignLive (signers@signer-gov.esignlive.com). In that email, click "Go to Documents".
Read the disclosures and click the Accept button at the bottom.
Complete the Declaration of Change form and upload your documents.
Finished? Click Confirm. Your form is now complete.

Submit Declaration of Change
NAICS Code/Descriptor Request Form: To request additional NAICS codes and/or descriptors, please complete the steps below.
- Download the NAICS Code/Descriptor Request Form.
- Open the downloaded form, fill it out (and save it), and gather your supporting documents.
- Print the completed form, sign it, scan it, and email the signed form along with its supporting documents to DOT.SDBE@dot.ohio.gov.
- Be sure to save a copy of everything you submitted for your files.
Once the UCP receives your request, we try to let you know within 30 days if the request is complete. If it is, we try to make a decision on your request within 90 days from the date of the original submission. If the request is not complete, we will let you know what is missing. The review process restarts once you submit the additional information.
