Report a Roadway Defect or Damage Incident and Court Of Claims Information

Report a pothole, debris or other roadway defect to ODOT

ODOT is responsible for the State System of roads – which includes all Interstates, as well as U.S. and State Routes outside of municipalities. If you have noticed and/or have experienced damage to your vehicle on a State System roadway as a result of a pothole, debris on the road, etc., please use the form below to send ODOT information about the location and incident. Your information will be forwarded along to a local ODOT office for attention.
If the identified location is on a local or municipal roadway and not on the State System, you should first try to contact the specific agency with jurisdiction for that location (for example: city, county, township, etc).

All ODOT maintained roadways (Interstate, US, or State Routes - use the selector above) have a route number. Any local street, avenue, boulevard, etc. with a name (and no other route number designations) is NOT under ODOT’s maintenance jurisdiction and notices/claims should be made with the responsible agency (municipality - city, township, village; or county) where the incident occurred.
Once your report is received by the ODOT District, you will receive an email or phone call letting you know who has maintenance jurisdiction:

3. Other Agency: (Municipality or County)
With information to file a claim with the appropriate party. Please allow one (1) to two (2) business days for ODOT District to reply to your report. Thank you.
