LPA Monitoring Survey

 

Project seems to be progressing well.  All poles have been erected on the east side of Clinton St. including light heads.  All lights on signal poles in place.  Contractor was working on wiring from pull boxes into light poles.  I looked at the pre-pour samples of porous concrete.  The concrete looks good.  The city has 3 choices of colors which a committee will decide.  The colors are brown, green and red.  I reviewed the documentation for this project and it looks good.  The city may want to add contractor’s equipment to their diaries.

Project seems to be progressing well.  Some items are ahead of schedule.  The contractor is now working on the west side of Clinton St. boring conduit and pouring pole foundations.  Poles for west side should be delivered in 7 weeks.  Bridge poles to be delivered April 4-11.  Base covers are not presently available and contractor is working on a solution to get the correct covers.  There are questions about which concrete repair areas are paid for and which the contractor is responsible for.  City of Defiance has decided not to stamp the porous concrete and will receive a discount from the contractor.  They have chosen the brown color for the porous concrete.  The benches should be delivered 5-7 weeks.  City of Defiance has decided on Ginco trees.  This was a good progress meeting discussing many problems throughout the project.

Visited the project today to go over what had been completed last year and what needed to be completed this spring.  Most of the paving work is complete with the exception of the far east of the project and connections to drives.  May need to mill out some pavement on the west side of Simon St. as this appears to be rough and not uniform.  Sidewalks are complete but need backfilled with topsoil and seeded.  Other small items need completed and the city inspector has a punch list for the contractor to complete.

Project seems to be progressing well.  Some items are ahead of schedule.  Contractor is pulling wires for light poles on the west side of Clinton St.  All the foundations and pull boxes are in place on the west side of Clinton St.  Contractor will begin erecting light poles nest week.  Contractor will begin reconstruction on the east side on Clinton St. next week which will include removal of pavers and tree stumps, repair sidewalk and curbs, and placement of porous concrete.

Some work has been completed this spring.  Mostly grading and sealing sidewalk joints.  I talked with Phil today about the revised completion date and completing this project this spring.  We discussed that the village should put the contractor on notice that they are on liquidated damages and they should submit a revised schedule to complete the project this spring.  There are contract items that need to be completed above the normal punch list items i.e. Paving and grading on the east end of the project, placing asphalt in drives, brick for handicap ramps, seeding and raising manholes.

Project seems to be progressing well.  Some items are ahead of schedule.  The contractor is working in the 100 block of Clinton St placing concrete patches and curb replacement and in the 500 block removing brick pavers.  The concrete and porous concrete and pavers are complete in the 300 block.  The light poles in the bridge are now erected.  I was not able to meet with Dave Pracht today because he was in a meeting.

Structural Steel for railroad bridge has been erected.  Excavation continues for Vine St. underpass. Porous backfill, water proofing and backfill around abutments continues.

I stopped by the project today and the contractor has begun to place new sidewalk on the east side of Cable Rd. between Spring and Market St.  Concrete was being placed from the road using flaggers and traffic seemed to be moving well.  The contractor has laid out the location of sidewalk to be placed north of Market St. on the east side of Cable Rd.  I am not sure of the official start date of this project as was not informed when they started.  It appears that the contractor has been there a few days as there are some handicap curb ramps already installed.

Project began yesterday 5/24/11by placing signs and delivering material.  Contractor is placing 8” tile on the northwest corner of the project.  Electric lines on the north side of the road need to be relocated for placement of piling.  No inspector on the job today.  He had been on the job earlier.

Contractor is placing 8” tile and Catch basin in the SW quadrant of the job.  Erosion Control items have been delivered and need to be placed.  I asked Jim to notify me of major work items and he needs to work with George Woolley on documentation.

Visited project site. Contractor working placing Bushes in the SE quadrant around the square.  Punch list items need to be completed.

Project seems to be progressing well.   Finished pouring pervious concrete in the 600 block of Clinton St. on the east side.  Work mostly complete in the 100, 300 and 500 blocks on the east side of Clinton St.  Trees not planted in the 200 block east side.  Contractor working on removal on the west side of Clinton St in the 100 block.

  Contractor has removed the old steel truss structure and abutments.  They have excavated for both footers.  They are ready to begin driving piles but they must wait until electric is moved which should be done this week.  They have placed some embankment on the east side of structure for the roadway.  I asked Jim Henf to make sure the embankment is up to subgrade 200’ behind the abutments before beginning to drive piles as per pg 13/28 of the plans. 

Visited project site.  Project is mostly complete.  Should be finished by the end of next week according to Mike Laderer.  Punch list items need to be completed.

Contractor began work on Thursday June 15th.   Contractor has removed the old steel truss structure. Steel is off the job.  Removing forward abutment today.

Contractor working on backfilling asphalt and concrete drives.  Asphalt, concrete drives and curb ramps are complete.  Project should be complete by the end of the week.

Project seems to be progressing well.  Contractor working in the 200 block on the west side of Clinton St. on removal items and in the 500 block west on placing pervious concrete.  They are also working in the 400 block on the east side of Clinton St. on items for the city including storm and sanitary sewer separation.  The contractor will probably have to remove the pervious concrete in the 100 block on the east side due to raveling.  I performed a documentation review today with Dave Pracht and the documentation looks good.  Dave is aware of everything that is going on pertaining to this project.  He is doing a nice job.  I did suggest that he put together a material file containing the supplier, quantity and the reference.

Full depth repair is complete.  Contractor is paving the patch areas today.  Milling is complete at structures.  Contractor will pave the entire project this week.

Contractor has completed this job and appears to be ready for a final inspection.  I will contact the county to make sure.  This is a good looking job.

Contractor is placing berm stone in the east bound direction-should finish paved areas today.  They still have a few miles of paving to complete in the westbound lane at the west end of the project.  Areas that are paved look good.

No work on this project today.  It appears that no work has been completed since steel truss was erected.

The city held a progress meeting today.  There appear to be very little complaints from the public to the work being performed which is supposing due to the nature of the work and the disruption to the property owners.  The waterline is 99% complete and the storm sewer will be complete by the end of the week.  AT&T appears to be holding back construction a bit as they were to be out of the way by Sept 1st and still working.  They should be complete by the end of the week.  This will allow the contractor to begin milling the road on Sept 26th.  I have advised the city and contractor to get their schedule updated and revise the completion date reflecting the delays so far due to the utilities.  The contractor still believes they can finish the project by the original completion date but I think it would be advisable to revise the completion date now.  A quarterly report for the DBE goal is due to the city by the end of the month.  The city will be testing the trenches for compaction and lining up testing for the subgrade and agg. Base soon.  I performed a documentation review today with Nathan and everything looks to be in good shape.  Pay items are well documented and they are keeping track of materials as they arrive on the project.

Contractor is pouring concrete for the backwall and wingwall on the east end of the structure.  They are also placing embankment on the west side roadway.  The prestress concrete box beams are now in place and the structure looks good.  The county should make sure that the contractor places all erosion control according to the SWPPP as soon as possible.  There appears to be an area on the east side that could use some ditch checks.  Compaction testing for embankment is not being performed again today.  I will review documentation to see if any compaction testing has taken place on this project. 

The contractor is using a mill to remove the roadway starting at Blanchard St.  They stared this work today.  Things seem to be progressing well but not sure they will have all work complete by the end of the season.  All utility work was complete on September 21st which is 3 weeks late.  This did slow the contractor down but they plan on finishing this year.  The city inspector should make sure erosion control around new catch basins where possible.

Contractor is cleaning the deck to prepare for waterproofing tomorrow.  They are placing Erosion Control on the east side of the structure.  They are preparing to place embankment on the west side.  The embankment will be tested by PSI tomorrow.  Brion Rhoades with the Allen Co. Engineers Office was on the project today.  Project appears to be on schedule for completion by November 1.  This is a good looking project.

Contractor is replacing the rail on Swaney Rd today near SR 81.  They are using steel posts and plastic blocks for this project.  The work looks good.  They may want to place some road work signs at the ends of the project.

Contractor is placing the 304 Agg Base on the west side roadway.  Waterproofing on the structure is complete.  Paving is going to take place next week on Tuesday.

Contractor was removing forms from deck today.  Deck looks good.  Top soil is now in place.  Need to get erosion control in place as soon as they can.  304 Agg base is complete.

Contractor is placing guardrail on bridge and at the ends.  All of the asphalt is in place and the striping is complete.  The project should be complete by the end of next week.  Erosion control needs to be put in place after seeding.

The City had a progress meeting today but I was not able to attend.  I met with Nathan and Chad after the meeting to discuss the project.  The contractor is scheduled to start paving the base on the project at the Melrose intersection beginning tomorrow.  The asphalt contractor will remain on the job after that until the job is finished.  The striping will need to be temporary as it will be too cold to place Thermoplastic stripes this late in the season.   The job will not be complete this year but the hope is to have everything paved and have full use of the roadway section.  The contractor will continue to work on the sidewalk and drives until weather no longer permits.  The city has discussed with the contractor the protection of any hazards that will be left through the winter and will follow up to make sure all is safe.  The City should agree on a revised completion date with the contractor to allow for the finishing of the project in the spring and incorporate the delays that occurred on the project for extra work and utilities.   

Project appears complete.  Someone needs to remove the signs from the west end of the project.

Ribbon cutting ceremony and road opening scheduled for this Friday, November 18, 2011 at 11:00 A.M.

It appears that much of the construction has slowed down for the winter.  The contractor was on site working on sidewalks and drives.  Most of the sidewalk and drives are in place from Blanchard St to Foulke.  All the drives on Crystal Ave appear to have access for the winter.  There will be work to complete this spring as this project will not be complete yet this year.  Contractor will want to cover all exposed dirt areas with straw for the winter to avoid erosion.  Grading should be done in all areas to avoid sudden drop-offs from sidewalks.

Contractor working today on sidewalks and drive approaches north of Foulke and seeding south of Foulke.  The south end of the job seems to be coming together nicely.  There are some manholes that will need to be adjusted to grade.

JDR working on 3 manholes at station 8+42.2 station 8+43.3 and 8+44.6. Laying 48” storm sewer to the west. Backfilling with hoe pack. Talked to Phil on T-E-24 concerning materials.

The contractor is seeding north of Foulke.  The seed south of Foulke has not yet started to grow.  Mainly just weeds.  The city should keep an eye on this area to make sure grass in established soon.  Most of this has been seeded over 2 weeks.  Most of the concrete walk and drives are complete with a few areas north of South Hill Trial to be completed

No work on the project today.  The contractor has placed the curb and gutter on both the north and south side of Spring St. from Collett St. to Jameson.  Continued sidewalk and gutter removal from Main St. to Collett.  Electrical contractor on site working on relocations at Elm and McDonald.  The stone grade for the curb and gutter work from Main to Collett does not appear to be compacted.  Please make sure there is a compactive effort before the placement of curb and gutter.

Most of storm sewer is complete. Going to start water lines 5/3/2012 No delays at this time.

Hume Supply not on the job today.  Olgesby on the job working on drive approaches.  Most of the curb and gutter on the south side of Spring St. complete.  Some of the Curb and gutter on the north side complete.  Work continues on Elm St to relocate the curve.  Traffic control seems to be functioning well.

 Working on the GRS north abutment three rows completed. Talked with Bob on some safety issues concerning hard hats and vest. I told him to make the contractor aware of the issue and document it.Also talked to him on a sub hauling dirt within a mile of the project concerning prevailing wages. This issue should be addressed to Robert Habern EEO Coordinator.

  Pre-construction meeting held today at the Village of Antwerp Everything in Appendix A was discussed. Start date is schedule for the week of 05/30/2012.

   Removing some trees and marking out some utilities out. Ebony coming in 5/31/2012 working on pavement planning.

JDR has completed all of the storm sewer and sanitary sewer lines. All of the water line is complete. Going to start under drains 06/07/12. Everything seems to be on schedule.

VernonNagel removed deck and beams. Concrete sawing on the rear abutment down to the beam seat. Excavating around the forward abutment. Working on areas to be patched on the rear pier.
